                                        MULTIPLE IDENTITIES QUESTIONNAIRE

 

          The following questions are part of a study of identities. Previous research has generally focused on only one or two identities at a time. This research explores multiple aspects of various identities that people may have. Some questions may raise unpleasant memories or may cause some participants to feel uncomfortable. On the other hand, participants may find the questions thought-provoking and interesting.

          In order to participate in this research you must be at least 18 years of age, so that you can legally consent. It should take about an hour to answer the qeustions, so begin when you have enough time to complete them. Your participation is completely voluntary. If you decide to participate, your answers will be kept completely anonymous and you may terminate your participation at any time with no negative consequences.

          We would like you to try to answer every question. However, if there are any questions that you do not wish to answer, you may skip them. If a question does not apply to you, leave it blank. Please nswer the questions privately. If the questions raise issues which you would like to discuss with someone other than friends, feel free to call the number below or use one of the following campus resources: the Residential Life staff (907-4223), the Counseling Center (907-4239), or other Student Services staff (907-4233).
